Medical Deployment



Deploying can be very emotional and stressful time for Airmen. Keeping yourself medically prepared to deploy will help ease some of the tension. Below is some information to help you medically out-process the 673 Medical Group more efficiently.

Check you individual medical readiness status and complete required actions.
Log into the Air Force Portal, scroll down to the Featured Links, open the Fitness & Health Link, and select Medical Readiness and Deployment. At the top of the page there is a link that is labeled "My individual Medical readiness Status". This link will show you what you need to accomplish to be medically ready in the Action List field. If you are coming due or currently overdue for a Preventive Health Assessment (PHA) or Dental exam, please call 580-2778 to schedule an appointment. You must be current by the Estimated Deployment Date (EDD)

Accomplish your DD 2795 Pre-deployment Health Assessment (DHA) within 120 day of your report no later than date (RDD).
To accomplish this form log into the Air Force Portal, scroll down to the Featured Links, open the Fitness & Health Link, and select Medical Readiness and Deployment. The page has a link labeled "Pre-Deployment Health Assessment Form" (DD From 2795). Select this link and follow the instructions to complete. Once completed, please call 580-2778 to schedule a DHA appointment.


Attend the Mental Health Brief as soon as you are notified of your deployment.
The briefing is required for all deploying members. It takes place in the Airmen and Family Readiness Log Cabin on Tuesday @0900. Members that will be receiving Hazard Pay are also required to accomplish the Automated Neuropsychological Assessment Metrics (ANAM). Your UDM will schedule this appointment.

Advanced preparation for a deployment will alleviate some stress and potential problem that may arise prior to out processing. Completing these steps in a timely manner will give you more time to spend with your loved ones prior to your deployment.
